Identification Bracelet

The Senior Citizen Commission and Council in cooperation with the Oak Lawn Police and Fire Department have an Identification Bracelet.

Oak Lawn residents 50 years of age and older can obtain a personal I.D. bracelet at the Senior Center, 5330 W. 95th Street.

You will be required to fill out an information sheet before you purchase your bracelet. This information will then be stored on the Emergency Communications Department computers.

You do not have to have a medical problem to purchase one of the bracelets. The purpose of the program is to obtain vital information in an efficient manner in case of an accident or emergency.

THE COST OF THE BRACELET IS $7.00.

Adjustments can be made to the bracelet, to remove or add links.

These adjustments can be made at Reichman Jewelers, 4439 W. 95th St. There is no charge for this service if you are a member of the Oak Lawn Senior Center.
